Ternion was established in 2015 in Phoenix as a group of physicians’ whose goal is to deliver better care for patients across the post acute continuum.

Use our experience and advancements in technology and communication to allow for seamless transition of care from the hospital to the post acute setting. Whether that is a sub acute facility, acute facility or home, allow patients to feel confident they will have quality care and all of their physicians including their primary will be fully up-to-date.
